Registration Forms to Download as Needed
Drug Authorization form (pdf) - If your child will need to take any kind of medication during the camp day you will need to complete this form.
**Don’t forget to plan ahead if your child will need medication for the Overnighter.
Scholarship Application (pdf) – Families requesting additional financial assistance will need to submit this completed form along with the Camper Application.
LIT Program Forms
LIT Conduct Agreement (pdf) – All LITs must sign and return a copy of the Conduct Agreement form prior to participation.
LIT Reference Form (pdf) - First-time LITs will need to have 2 reference forms completed and returned to camp.
LIT Supplimental Application (pdf) – Each LIT will need to complete this supplimental application in addition to the regular registration form completed by the parent.
